Certification of Identity – Jabatan Imigresen Malaysia
This Certificate of Identity is issued to stateless people who are Permanent Residents of Malaysia for the purpose of travelling abroad.
Validity of Certification of Identity
Five (5) years from the date of issue.

- Form-IM.45
- Two (2) passport size photographs of applicant (3.5 cm x 5.0 cm)
- Previous/Old Malaysia Certificate of Identity (for renewal)

Below 18 years but married:

- Marriage Certificate

Below 18 years:

- Parent’s Identification Card/Entry Permit/Passport
Below 18 years – Parents (Deceased):
- Identification Card (12 years and above)
- Entry Permit
- Birth Certificate
- Parents’ Death Certificate
- Statutory Declaration of Guardianship
Below 18 years – Parents are divorced and cannot be contacted:
- Identification Card (12 years and above)
- Entry Permit
- Birth Certificate
- Either Parent’s Identity Card/Entry Permit/Passport
- Court’s Order for Child Custody
Below 18 years – Adopted Child:
- Identification Card (12 years and above)
- Entry Permit
- Birth Certificate
- Identification Card of Legal Guardian
- Certificate of Adoption
