New and replacement of MyKad application can be done at any of Malaysia’s National Registration Department (NRD) or Jabatan Perkhidmatan Negara (JPN) as referred in Malay.

Applying Identity Card for 12 Years Old Child 

Application Procedure

Applicant is required to go to the NRD counter with the following supporting documents (Original & copy):

• Birth certificate 
• Identity card of mother or father or guardian 
• Certificate of adoption 
• Form W Declaration of oath (guardian) 

According to Regulation 3, National Registration Procedures 1990 (Amendment 2001) all persons aged 12 must change their identity card. 

Conditions of Application

Applicant must be 12 years old. Applicant must be present with the sponsor (mother or father or guardian)

• Fill up JPN.KP01 form (obtainable at NRD counters) 
• Cash payment

Replacement Of Identity Card For 18-Year-Old/Change Of Address/Damaged/Defective

Application Procedure

Applicant is required to go to the NRD counter with the following supporting documents (Original & copy): 

• Identity card 
• Electric bill/water bill 

According to Regulation 18, National Registration Procedures 1990 (Amendment 2001) all persons aged 18 – 25 must change their identity card.
