How to Apply for Late Registration of Birth in Sabah?

Learn how to apply for late registration of birth in Sabah. Late registration of birth refers to registration made more than 42 days after the date of birth of child in Sabah.

Application Conditions

Eligibility of the Applicant, Sponsor or Informant:

  • Parent;
  • If the father is not available, then the mother or a sponsor with knowledge of the birth who is 10 years older than the child ;
  • If the father is not available and the mother has died, the application may be submitted by two(2) sponsors with knowledge of the child, who are both aged 18 and above and are at least 10 years older than the child;
  • If the newborn is found exposed and its birth was not registered, then the application may be submitted by two(2) sponsors who are social welfare officers or persons given custody of the child in accordance with the Child Protection Act 1991;

Application Procedure

Applicant for late registration of birth can be made throughout Sabah by bringing along the necessary documents to any NRD office near the applicant’s place of residence

Required Documents (Home Birth)

How to Apply for Late Registration of Birth in SabahChild’s Documents (Originals and Copies):

  • Birth registration search form N1 (for births from 2002 onwords);
  • Birth registration form (Form A) that is completed;
  • Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/ doctor (originals and copies). If unavailable, a statutory declaration is required;
  • Recent full-length 4R photograph (one(1));
  • Recent passport-sized photograph (one(1));
  • Certification of birth from a headman or village chief or Member of Parliament or Member of State Legislative Assembly for the constituency where the child was born;
  • Extract of school register or schooling and aged seven(7) and above, or alternatively, certification form a headman or village chief or Member of Parliament or Member of a State Legislative Assembly for the constituency where the child resides, that the child has never been schooled and aged seven (7) and above (if any);
  • Statutory declarations of two (2) witnesses with knowledge of the birth

Parents’ Documents (Originals and Copies):

  • Identity cards or entry permits or passports or death certificates (parents or informant or witness or sponsor)(originals and copies);
  • Marriage document or divorce certificate or death certificates of parents (originals and copies);
  • Recent passport-sized photograph of each parent (one(1))

Sibling’s Documents (Originals and Copies):

  • Birth certificate or identity card, if any (original and copy);
  • Recent passport-sized photigraph of the sibling from the same parents (one(1));
  • Family information forms BMK-81 and BMK-81A

Sponsor’s Documents (Original and Copies):

  • Statutory declaration JPN.LM27 of two (2) sponsors with knowledge of the birth;
  • Identity card or entry permit or passport or death certificate of the informant or witness or sponsor (original and copy)

Required Documents (Birth in Government/ Private Hospital or Clinic)

Child’s Documents(Originals and Copies):

  • Birth registration search or extract form –Form N1;
  • Birth registration form A;
  • Confirmation of birth form;
  • Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/doctor (originals/copies);
  • Recent full-length 4R photograph (one(1));
  • Recent passport-sized photograph (one (1));
  • Extract of school register or school certificate with endorsement from the headmaster or school principal if the child is schooling and aged seven (7) and above, or alternatively, certification from a headman or village chief or Member of Parliament or Member of a State Legislative assembly for the constituency where the child resides, that the child has never been schooled and is aged seven (7) and above (if any)

Parents’ Documents (Originals and Copies):

  • Identity Card or entry permit or passport or death certificate (parents. informant, witness or sponsor)(originals and copies);
  • Marriage document or divorce certificate or death certificates of parents (originals and copies);
  • Recent passport-sized photograph of each parent (one (1))

Sibling’s Documents (Originals and Copies):

  • Birth certificate or identity card, if any (original and copy);
  • Recent passport-sized photograph of the sibling from the same parents (one (1));
  • Family information forms BMK-81 and BMK-81A

Sponsor’s Documents (Originals and Copies):

  • Statutory declaration JPN.LM27 of two (2) sponsors with knowledge of the birth;
  • Identity card or entry permit or passport or death certificate of the informant or witness or sponsor (original and copy)
