Issued to a foreigner who hold acceptable professional qualifications or specialist skills and enter Malaysia to take up professional work for short term period not exceeding twelve (12) month. Applicant must be outside Malaysia at the time of application.

- Aged 40 years old and above for Gurukkals, Granthis, Dharma teachers and priest
- Aged 35 years old and above for religious musician
- Aged 30 years old and above for religious sculptor

- Supporting Letter from Malaysia Hindu Sangam, Malaysia Buddhist Society, Malaysia Youth Buddhist Society, Malaysia Gudwara Council, Ministry of Human Resource
- Form 12
- Form IMM 38
- 2 recent passport size photograph
- Personal Bond
- Photocopy of Passport
- List of Committee Members (name, position hold in the committee, profession, identification card, address and signature)
- Local newspaper advertisement
- Locality plan
- Photocopy of land title
- Certified approval Letter to build / renovate the temple from local authority
- Recent temple photograph
- Name list of recent local and foreign workers
- Certified copy of Registrar of Society
- Financial statement
- Supporting letter from certified architect regarding sculpture work
- Proposal work progress for sculpture work
