How to Apply for Extract of Birth Certificate in Sarawak?
Learn how to apply for extract of birth certificate in Sarawak. Application by an eligible applicant to obtain a copy of the birth register by paying a prescribed fee. Extract of the Birth Certificate can be made only at the State NRD Office and Putrajaya NRD.

- Birth Certificate owner;
- Parent of Birth Certificate owner;
- Spouse of Birth Certificate owner;
- Child or legally adopted child of birth certificate owner;
- Sibling of Birth Certificate owner;
- Law firm;
- Statutory body;
- Government agency
Required Documents
CATEGORY 1: PERSON IN CATEGORIES 1 TO 5
- Form X that is completed;
- Identity card or identification document (original and copy);
- Copy of Birth Certificate of owner (if available)
CATEGORY 2: PERSON IN CATEGORY 6
- Form X that is completed;
- Official letter from a law firm requesting search or extract of the register;
- Letter of appointment from client (applicant);
- Identity card or identification document of client (applicant) (original and copy);
- Power of attorney to oversee the application (if applicable)
CATEGORY 3: PERSON IN CATEGORIES 7 AND 8
- Form X that is completed;
- Official letter from a statutory body or government agency to apply for a search or extract of the register;
- Power of attorney to oversee the application (if applicable)
Payment
Application fee is RM5.00. Government agencies are exempted from payment.
